We believe it is essential to tour a facility to explore how well it meets your needs or those of your loved one. To learn how we can meet the individual needs of you or your loved one and to arrange a personal tour, please call (502) 459-1400.
Upon admission, a personal care plan will be developed by our interdisciplinary team along with a guide to how the care will be provided to the resident. This plan will be monitored and modified as necessary throughout a resident's stay, whether long- or short-term. Family and caregivers’ involvement in this process is highly encouraged and recommended.
